Healthcare AEO FAQ

AEO, AI search visibility, pricing, timelines, fit, and what the audit covers — answered directly by the person who leads the work.

AEO Basics

6 questions

What is Healthcare AEO?

Healthcare AEO (Answer Engine Optimization) is the practice of structuring your online presence so that AI systems — ChatGPT, Gemini, Claude, Perplexity, Google AI Overviews — can find, understand, and recommend your practice when patients ask relevant questions.

How is AEO different from SEO?

SEO focuses on ranking in Google's search results. AEO focuses on being cited in AI-generated answers. A healthcare practice can rank #1 on Google and still be invisible to AI if it hasn't structured its data for AI consumption.

Why do healthcare practices need AEO?

Patients are increasingly using AI tools instead of Googling. AI gives one answer — not ten links. If you haven't structured your practice for AI consumption, AI will recommend a competitor instead.

Which AI platforms do you optimize for?

ChatGPT, Gemini, Claude, Perplexity, and Google AI Overviews. Those are the five systems healthcare patients use when they research a practice before booking, and every audit, implementation, and report covers all five.

Can I keep my SEO agency and add AEO?

Yes — good SEO and AEO share foundations (structured data, consistent listings, authoritative content), but AEO is not SEO with a new label. Ranking #1 on Google does not mean AI cites you, and an SEO agency that has not rebuilt its reporting around per-provider, per-service citation data is not doing AEO. Keep your SEO firm if they're good at SEO; add AEO work that is measured on AI citations, not rankings.

How long does AEO take to show results?

Measurable citation lift usually takes 60 to 90 days; full AI visibility — structured data, cross-platform consistency, authority signals — typically takes 3-6 months. Initial improvements in data consistency can be indexed within weeks. The order matters: consistency first, then the authority signals that take longer to accumulate.

How AI Search Works

8 questions

How do AI systems decide which practice to recommend?

When a patient asks an AI a question, the system retrieves sources it can parse, synthesizes an answer, and names the businesses those sources support. It favors practices it can verify: consistent facts across your website, Google Business Profile, and directories; valid structured data it can read without guessing; and third-party consensus — reviews, citations, media mentions — showing multiple sources agree. You cannot pay to appear in the organic answer; the only lever is making your practice the most verifiable, most corroborated entity in your specialty and market.

Does ranking #1 on Google mean AI will recommend me?

No. In Ahrefs' July 2025 test of 15,000 queries, only about 8% of ChatGPT's in-text citations ranked in Google's top 10 for the same query; across AI assistants the average was about 12%, with Perplexity the outlier near 28% ([Ahrefs](https://ahrefs.com/blog/ai-search-overlap/)). A Google ranking is evidence about Google's system, not about the AI answer — you can hold the top result for your procedure in your city and still go unmentioned when a patient asks ChatGPT the same question.

Do I need a new website or schema markup for AEO?

Almost never a new website. What most practices need is schema markup — structured data that tells an AI what your practice is, who your providers are, and what you do — added to the site they already have, plus content written so the answer comes before the elaboration. Pages with structured data earn roughly 2.5x more AI citations in the citation research Stackmatix summarizes ([CJ Online, via Stackmatix, 2026](https://www.stackmatix.com/blog/aeo-statistics-trends)). A rebuild only enters the conversation if your current site can't carry schema, blocks crawlers, or can't be edited at all.

Do AI crawlers read my website — and can I block them?

Yes, if you let them. Systems like GPTBot, OAI-SearchBot, and ClaudeBot fetch public web pages to build and update the knowledge those answers draw from. You can block them in your robots.txt — and if you do, you have opted out of being recommended. We check your robots.txt in the audit; an accidental block is one of the fastest fixes there is.

How do Google Business Profile and reviews affect AI recommendations?

Both matter, differently by platform. Google AI Overviews read your Google Business Profile directly — an incomplete or contradictory profile is a direct visibility loss there. For ChatGPT, Perplexity, and Gemini, your profile and your reviews are among the third-party sources those systems cross-check: reviews are consensus evidence that multiple sources agree you exist and are worth naming. "Fix first" means claimed, complete, consistent — name, address, phone, hours, and services matching your website exactly, with no contradictions between locations.

What should I do if AI says something wrong about my clinic?

There is no correction form that fixes the underlying data. Individual answers on some platforms have a feedback button, but that adjusts that one response — it does not repair what the system read. The durable fix is the source layer: correct your Google Business Profile, your directory listings, and your own site so every public fact agrees, add structured data stating the facts explicitly, and let the engines re-crawl. Re-crawls run on the platforms' schedules, so expect weeks to months, not hours. We track what the engines currently say about you per provider and per service, so a new wrong answer shows up in monitoring rather than in a patient's booking decision.

How often does what AI says about you change?

Continuously. Answers are synthesized at ask-time from whatever the system retrieves then — the same question can return different sources depending on phrasing, location, and what was recently crawled. Industry data on citation churn shows the set of sources AI cites can shift substantially month to month. This is why visibility is a monitoring problem, not a one-time fix: what AI said about you in March is not a guarantee about October.

Which sources do the AI platforms actually read?

Differently by platform, which is why visibility on one does not carry to the rest. ChatGPT's search draws on Bing, and tests suggest it sometimes uses Google as a fallback. Perplexity crawls the live web in the moment. Google AI Overviews run on Google's own index plus your Business Profile. Gemini draws on Google's index and its training data. Claude combines web retrieval with training data. Each system weights your website, directories, reviews, and media mentions differently — the audit maps which sources each engine is actually pulling for your specialty.

Healthcare Fit & Compliance

12 questions

Who does the work?

Sandeep Kumar, founder of Pager Studio. Pager Studio is a founder-led AI visibility partner for fertility clinics, built for practices that want direct specialist oversight rather than an agency production line. Every engagement is personally assessed, prioritized, and overseen by Sandeep — no team, no outsourced work, no junior copywriter drafting your strategy. That is also why we take on a limited number of clients at a time.

Do you work with non-healthcare businesses?

No. Pager Studio works exclusively with healthcare practices — primarily fertility clinics. This specialization allows us to build deep expertise in healthcare AEO rather than spreading ourselves across industries.

Which specialties do you work with?

Primarily fertility clinics — IVF & Fertility is the center of the practice. We also selectively work with a small number of other high-consideration healthcare practices: cosmetic dermatology, dental implants, LASIK, hair restoration, and plastic surgery.

Do you guarantee citations, rankings or patient volume?

No, and you should be wary of anyone who does. No one can control how an AI platform answers a query. What we guarantee is the work: the audit gets done properly, the foundation is built correctly, and you receive transparent monthly reporting on citation movement across all five platforms.

Do you need access to patient data?

No. Never. We do not collect, store or process patient data, medical records, or patient images at any point. Our work covers your public web presence — website, directories, structured data, and the signals AI systems read.

Do you publish client results or case studies?

No. All engagements are covered by a formal NDA. We never publish client names, metrics, or the strategies we build for them. If you want proof, claim your free snapshot — a basic AEO look at how ChatGPT sees your practice, emailed to you. That's a free preview, not the paid $5,000 Audit — it's a quick look at your ChatGPT visibility, not the full provider-by-provider, multi-prompt diagnostic across all five platforms.

Where are you based, and do you work with US practices?

Pager Studio is based in Coimbatore, Tamil Nadu, India, and works with US healthcare practices remotely. All work follows the same remote process regardless of the practice's location.

Can I make claims about treatments in AI-facing content?

We don't write or approve treatment claims — your compliance and legal team does, under the rules that already govern your advertising (state medical boards, FDA, FTC). What AEO changes is extractability: an AI repeats what your public pages say, often verbatim. So the bar for anything published is simple — if you are not comfortable with a sentence being read back to a patient by ChatGPT word for word, it should not be on the page. Our audit flags content that is likely to be extracted and how, but the claims themselves stay yours.

Do I need HIPAA compliance for AEO?

The work itself never touches PHI. We do not collect, store, or process patient data, so there is no protected health information flowing to us and no business associate agreement required for our engagement. We work on your public web presence — the pages, listings, and structured data anyone can already see. HIPAA obligations on your own site (appointment forms, patient portals, anything collecting health information) are your compliance program's domain, not ours; we can flag publicly visible issues we notice, but we don't provide legal or compliance advice.

Is AEO worth it for a solo or single-provider practice?

It depends on one number: the lifetime value of a single patient in your specialty. In high-consideration fields — IVF, plastic surgery, LASIK, dental implants — one patient frequently justifies the entire foundation, because patients in those categories research for weeks and book with whoever the research points to. If your economics don't clear that bar, we'll say so up front rather than sell you an audit that can't pay for itself. Being small is not a disadvantage in AEO: a single-provider practice with clean data often beats a multi-location group whose listings contradict each other.

How does AEO work for multi-location or multi-doctor practices?

Each location and each provider is its own entity in AI's eyes, so the audit, the fixes, and the reporting are all per-location and per-provider — never one practice-level score. Every location needs its own complete Google Business Profile, its own consistent name-address-phone across directories, and structured data that distinguishes it from the other sites. Providers need entity pages AI can parse: credentials, procedures, locations worked. Inconsistency between your own locations is one of the most common reasons AI declines to name any of them.

What makes healthcare AEO different from general AEO?

Three things. First, the stakes: a wrong recommendation in a high-consideration specialty is not a lost t-shirt sale, it's a patient making a decision they can't take back — so AI engines apply extra scrutiny to health sources, and thin content gets filtered out faster. Second, the entity layer: patients ask about providers, not brands, so per-provider visibility matters in healthcare in a way it doesn't for a SaaS company. Third, the rules: treatment claims, before-and-after content, and outcome statistics carry regulatory weight that a general AEO agency has never had to think about.

Pricing & Engagement

8 questions

How much does the AEO Audit cost?

$5,000, fixed. Delivered in 4-6 weeks. No obligation to continue afterward.

Why is the audit $5,000?

A thorough AEO audit requires manual analysis across multiple AI platforms, directory verification, structured data review, and competitive analysis. The $5,000 fee covers all of this — and the full $5,000 is credited toward your 6-month foundation if you sign within 15 days of receiving your audit.

What is included in the Healthcare AEO Audit?

The audit covers your current AI visibility across ChatGPT, Gemini, Claude, Perplexity, and Google AI Overviews. We analyze data consistency, structured data coverage, content extractability, and authority signals. You receive a written report: your visibility per platform, what's inconsistent, what your competitors own, and the order to fix it in. The audit is $5,000 fixed and delivered in 4-6 weeks.

What does ongoing implementation cost?

We quote implementation pricing after your audit, based on your practice, specialties, and markets. There is no public retainer price — a single-provider clinic and a multi-location group do not cost the same to implement for, and we would rather quote accurately than publish a number that fits neither.

Is there a minimum engagement?

Yes — six months. That is the shortest term in which structured data, consistency, and authority signals actually compound into citation movement, so we don't offer anything shorter. The audit itself is a one-time $5,000 deliverable with no obligation afterward. After the 6 months, engagement is month-to-month: cancel any time with 30 days' notice, no penalties, no exit fee. The monthly fee covers strategy, implementation across all five platforms, and the monthly citation report — there are no separate change-order fees inside the scoped work.

What happens to the $5,000 if I continue?

The full $5,000 is credited toward your 6-month foundation if you sign within 15 days of receiving your audit. Outside that window, the audit fee is non-refundable — it paid for a completed deliverable.

What is not included in your services?

Paid ads, social media management, and web development. Pager Studio does AEO — audit, implementation, and monitoring for AI citation. We don't run ad campaigns (including the new ChatGPT ads), manage social accounts, or build websites.

Are there any hidden fees?

No. The audit is $5,000, quoted in writing before you pay. Implementation pricing is quoted after the audit, also in writing, before any work starts. The monthly fee covers the scoped work — there are no setup charges, no per-report fees, no surprise line items. If something falls outside scope, we flag it and quote it separately before touching it. Every cost is approved by you before it lands.

Process & Timeline

6 questions

What happens after the audit?

You receive the written report: your visibility per platform, what's inconsistent, what your competitors own, and the order to fix it in. If you want to proceed, the next step is implementation — strategy, technical optimization, and ongoing monitoring. Minimum engagement is 6 months.

What do you need from us to get started?

Access to your website (or developer contact), your Google Business Profile, and a list of your providers, locations, and the procedures you want visible. No patient data, ever. Most practices gather this in under an hour.

How do you report progress?

Monthly, with citation movement tracked across all five platforms — ChatGPT, Gemini, Claude, Perplexity, and Google AI Overviews — broken down per provider, per service, and per location. Not one vanity score for the whole practice.

What's the difference between the free snapshot and the $5,000 Audit?

The free snapshot is a quick, single-platform look at how ChatGPT currently describes your practice — prepared manually by Sandeep and emailed to you. It covers ChatGPT only. The $5,000 Audit is a separate, deeper engagement — provider-by-provider, location-by-location, multiple patient-intent prompts across all five platforms, directory and schema review, and a full competitor comparison, delivered as a written report in 4-6 weeks. The snapshot tells you there's a gap. The Audit tells you exactly what's in it and how to close it.

How do I know AEO is working?

One thing counts: your practice being named in the answers patients get, on the five platforms, for your specialties in your markets. We measure that as citation frequency across a fixed set of patient-intent prompts, per provider and per location, tracked month over month. Leading indicators — schema validity, directory consistency, profile completeness — move first, typically within weeks; citations usually follow in 60 to 90 days. A single "AI visibility score" with no underlying data is not proof of anything; the proof is being in the answer.

What can slow results down?

Three things, in practice. Site changes waiting on a developer or a CMS you can't edit quickly — schema can't go live if nobody can publish it. Content approvals bottlenecked at one physician who reviews once a month. And unknown conflicting listings: old addresses, defunct phone numbers, duplicate profiles from a previous marketing vendor, which the audit surfaces but which take time to unwind. None of these are reasons to not do AEO — they're exactly what the audit is designed to find before you're paying for implementation.

Choosing an Agency

6 questions

Is AEO just renamed SEO?

No. SEO optimizes for ranking in Google's link results; AEO optimizes for being cited in AI-generated answers. The signals overlap partially, but the measurement, the tactics, and the failure modes are different — a practice can rank #1 on Google and be completely absent from ChatGPT. Any agency selling 'SEO, now with AEO' without per-provider citation reporting is selling a rename.

How do I spot an AEO agency that's really just an SEO shop?

Watch what their report leads with. If the first page is keyword rankings and domain authority, with no per-provider citation data from ChatGPT, Perplexity, or Google AI Overviews anywhere in the deck, they are reporting on Google and calling it AI visibility. Other tells: they can't explain how an AI picks a source; they guarantee you'll be "#1 in ChatGPT" (no one controls model outputs — we don't either); or "AEO" appeared on their website within the last few months while the methodology section still reads like 2019 SEO. The fastest test: ask them to show a redacted citation report from a current client. If none exists, there is nothing to show.

What should I ask an AEO agency before hiring them?

Five questions, in this order. One: how do you measure results — the only acceptable answer is named citations per prompt set, per platform, per provider, tracked over time. Two: who does the work — if it's "our content team," ask who built the measurement. Three: can I see a redacted sample report. Four: what's excluded — ads, web development, social should all be named. Five: do you touch patient data — the only correct answer is no. An agency that answers all five cleanly is rare. An agency that deflects on question one is telling you everything.

Can I do AEO myself?

Part of it, yes. Adding schema markup, claiming and completing your Google Business Profile, and making your directory listings consistent are real fixes you or a competent marketing generalist can execute — our blog walks through the manual check for free. What's hard alone: running consistent patient-intent prompts across five platforms per provider per service and knowing which citation gaps actually matter versus which are noise; building provider-entity pages AI can parse without cannibalizing each other; and reading a wrong AI answer and tracing it back to the specific source that caused it. The audit exists so you don't pay us to guess — you pay for the diagnosis, and you're free to implement it yourself.

What should an AEO monthly report contain?

Citations, not rankings. Concretely: which patient-intent prompts were tested, on which of the five platforms, whether your practice was named, per provider and per location, compared to last month and to your two biggest competitors. Plus what work was done that month and what's planned next. If your report doesn't name a single prompt and a single platform, it isn't an AEO report — it's an SEO report with a different cover page.

Do you resell automated visibility tools?

No. Free and cheap visibility checkers exist — we've written about which ones are worth your time — and they're fine for a spot-check. They are not an audit: a screenshot of one score can't tell you which provider is invisible, which directory is contradicting your site, or which competitor is taking the citation you wanted. Our audit is manual, prompt by prompt, across all five platforms, per provider and per service. If an agency's deliverable is a tool screenshot, that screenshot is the ceiling of what you're buying.

Getting Started

6 questions

What do I get for free?

A personal AI visibility snapshot of your fertility clinic — what ChatGPT currently says about you, what it gets wrong, and where the gaps are. Sandeep prepares it manually and emails it to you. No call, no calendar, no obligation. If you want to talk further, you just reply to that email. The full end-to-end audit covers all major AI platforms.

How do I check if my practice is visible to AI?

Ask ChatGPT a question a patient would ask about your specialty in your area. If your practice isn't mentioned, AI either doesn't know you exist or doesn't trust your data enough to recommend you. Our free snapshot runs this check on ChatGPT; the full end-to-end audit runs it across all five platforms.

How do I get started?

Claim your free snapshot at /book. Sandeep prepares it by hand — how ChatGPT describes your practice — and emails it to you. Free, no obligation, no calendar. If you want to talk further, you just reply. This is a quick preview of ChatGPT, not the paid $5,000 Audit — which covers all major AI platforms and is a separate next step if it's a fit.

What happens if I do nothing?

AI keeps absorbing the research patients used to do in a search bar. Gartner predicted in February 2024 that traditional search engine volume would drop 25% by 2026 as chatbots take those queries, and BrightEdge measured AI Overviews already triggering on roughly 88% of tracked healthcare queries as of December 2025. The practices that get named in those answers over the next two years are the ones building the entity signals now. Doing nothing is a decision — it's the decision to stay out of the answer being given today.

Who on my team should own AEO?

Whoever already owns your web presence — practice manager, marketing lead, or you, if you're the one who answers the phone and signs the checks. During implementation we need roughly one to two hours a month from that person: approving content, confirming provider and location facts, and unblocking site changes. The strategy and the work are ours; the facts about your practice are yours, and someone has to be the source of truth for them.

How soon can we start?

Free snapshots are typically prepared and emailed within a week of your request. If it's a fit, the $5,000 audit starts right after and delivers in 4-6 weeks. There's no waitlist theater — we take on a limited number of clients at a time, and if the roster is full we'll tell you rather than string you along.
