Your SEO Agency Stuck an "AEO" Sticker on Their Invoice — Here's What They're Not Tracking
Most agencies selling "AEO" are SEO shops that renamed a package without changing the work. The test: ask for AI visibility by doctor, service, and location.

Quick answer: Most agencies selling "AEO" are SEO shops that renamed a package without changing the work. The test: ask them to show AI visibility broken down by doctor, by service, and by location. A real AEO partner has that data. A rebranded SEO shop will pivot to talking about "domain authority" instead.
TL;DR
- Fake AEO is SEO relabeled: same methodology, same keyword reports, a new invoice line. Most agencies marketing "AEO" are SEO shops that never changed the work.
- The test: ask for AI visibility broken down by doctor, by service, and by location. Real AEO partners have that data; sticker agencies pivot to "domain authority."
- Three or more of the 7 red flags — no citation dashboard, no per-provider tracking, reports still leading with keyword rankings — means you're paying for a sticker.
- SEO isn't dead, it just can't cover both: rankings and local visibility are SEO; doctor-, service-, and location-level presence inside AI answers is AEO. Make sure you're not paying twice for one.
What "Fake AEO" Actually Means
Fake AEO is SEO work relabeled as AI visibility work, with no change to methodology, tracking, or deliverables. The agency renames the package, adds a line item to the invoice, and keeps reporting keyword rankings as if they were AI citation data.
The pattern is consistent across the industry: an SEO firm notices patient search behavior shifting to ChatGPT, Perplexity, and Google AI Overviews, refreshes its homepage and service names, and starts charging more — without adding the tools, prompts, or platform monitoring that actual AEO requires. No credible count of how many agencies have rebranded exists — which is exactly why the tell has to be their reporting, not their marketing.
Why AEO and SEO Are Not the Same Thing
SEO gets a webpage ranked on Google. AEO gets a specific person, service, or location recommended inside an AI-generated answer. They share some inputs — structured data and clear content help both — but they optimize for different outcomes and are measured with different metrics entirely.
This distinction isn't cosmetic. In Ahrefs' July 2025 test, only about 8% of ChatGPT's in-text citations ranked in Google's top 10 for the same query (Ahrefs). Ranking well on Google is not evidence that AI recommends you — the two systems draw from different signals and return different answers.
Healthcare search has already shifted decisively toward AI-mediated answers: AI Overviews now trigger on roughly 88% of tracked healthcare queries (BrightEdge, December 2025 data). A practice invisible inside those answers is losing patients at the exact moment of decision, regardless of its Google ranking.
Sticker AEO vs. Real AEO
| What a "sticker AEO" agency reports | What real AEO tracks |
|---|---|
| Keyword rankings | AI recommendation rate, by platform |
| One visibility score for the whole practice | Per-doctor: does ChatGPT name Dr. X by name? |
| "Your site is visible" | Per-service: IVF vs. IUI vs. egg freezing, tracked separately |
| City-level SEO rank | Per-location: each clinic, each market, tracked separately |
| Traffic and clicks | Citations across ChatGPT, Gemini, Claude, Perplexity, and AI Overviews |
A rank report tells you your position on a Google results page. It cannot tell you that ChatGPT is currently recommending a competitor's physician for the exact procedure your practice is known for.
Why AI Visibility Has to Be Tracked by Doctor, Service, and Location
A single, practice-wide visibility score hides the three places most practices are actually losing patients. AI doesn't answer "is this practice good?" — it answers specific questions patients actually ask:
"Which IVF doctor in Austin has the best success rates?"
"How much does a hair transplant cost in Dallas?"
"Which LASIK clinic near me uses the newest technology?"
Each of those gets one answer — a named doctor, a specific service, a specific location — not a practice-wide summary. That means visibility has to be measured at the same granularity:
- Doctor-wise: A practice can rank well overall while AI still recommends a competitor's physician by name when a patient asks for a referral.
- Service-wise: A clinic can appear for a broad query like "fertility clinic Austin" and still be absent from the high-intent queries — "IVF success rates," "egg freezing cost" — that precede an actual booking.
- Location-wise: A flagship location can show up in AI answers while a second or third location is invisible, losing patients to a competitor twenty minutes closer.
An agency reporting one number for an entire multi-doctor, multi-location practice isn't doing AEO. They're presenting a traffic report with a new name on it.

7 Red Flags That Your "AEO" Retainer Is Rebranded SEO
Check your most recent invoice and monthly report against this list:
- No citation dashboard — rank tracking is present, but there's no data on how often ChatGPT, Perplexity, or Gemini cite the practice.
- No per-provider tracking — nothing answers "which of my doctors does AI recommend?"
- No per-service breakdown — visibility is reported for the practice as a whole, never for the specific procedures that generate revenue.
- No per-location analysis — a practice with three clinics gets one visibility score.
- "AEO" priced as a cheap add-on — under $1,500/month labeled AEO is almost always rebranded SEO.
- FAQ schema plus a summary box called "the strategy" — a plugin and a TL;DR box is a formatting change, not a methodology.
- The report still leads with keyword rankings — if page one of the monthly report is Google positions with no AI-platform data anywhere, the invoice changed and the work didn't.
Three or more of these? You're paying for a sticker.

The One Question That Exposes a Sticker Agency
Send your agency this exact question:
"Which of my doctors does ChatGPT recommend for [service] in [city] — and which of my locations show up in AI answers?"
A real AEO partner answers with data: the prompts tested, the platforms checked, which providers appeared, which didn't, and what changed since the last report. A sticker agency pivots to "domain authority," "content quality," or "SEO fundamentals feed AI anyway" — true statements that are not answers to the question asked.
If producing that answer takes them two weeks, they didn't have the data before you asked.
What Real AEO Costs
The average SEO retainer runs approximately $3,209/month (Ahrefs SEO Pricing survey). Dedicated AEO work is priced separately: $1,500-$15,000+/month depending on scope (Acromatico GEO Agency Pricing Index, 2026), with standalone AEO audits generally running $1,500-$12,000.
When that premium buys real AEO work, it funds things SEO retainers don't cover: entity structure per provider, schema for each service and location, directory consistency across every listing, and ongoing citation monitoring across AI platforms.
When it's rebranded SEO, the premium buys the same keyword report with "AI" added to the header — you fund the agency's rebrand, not your own visibility.
SEO Isn't Dead — It Has to Run Alongside AEO, Not Disguised as It
Google still sends patients, and local search still drives bookings — BrightEdge's healthcare data shows Google pulled AI Overviews entirely off local "near me" queries, dropping from 100% coverage in 2023 to 0% today, meaning Google Business Profiles and local rankings remain load-bearing.
The fix isn't replacing SEO with AEO — it's making sure you're only paying once for each:
- SEO covers: rankings, local visibility, site speed, backlinks, "near me" queries.
- AEO covers: doctor-level, service-level, and location-level presence inside AI-generated answers.
If an agency can't show these as two distinct deliverables with two distinct reports, the client is paying twice for one service.
Frequently Asked Questions
How do I know if my AEO agency is just doing SEO?
Ask for citation tracking across ChatGPT, Perplexity, and Google AI Overviews, broken down by doctor, service, and location. If the agency can only show keyword rankings, or needs weeks to produce AI data it has never reported before, the "AEO" label is cosmetic rather than a change in methodology.
Why does AEO need to be tracked doctor-wise, service-wise, and location-wise?
Because patients ask AI for a specific doctor, a specific procedure, and a specific place — never for "a practice" in the abstract. A single practice-level score conceals exactly which providers AI ignores, which services never surface in answers, and which locations lose out to competitors, so granular tracking is the only way to identify what's actually broken.
What does fake AEO typically include?
FAQ schema, a TL;DR summary block, and sometimes a blog post — the same content and link-building playbook already sold as SEO, applied to a new invoice line. It does not include per-provider entity optimization, per-location directory consistency, per-service structured data, or monitoring across AI platforms.
Is AEO just SEO with a new name?
No. SEO optimizes for ranking in Google; AEO optimizes for being recommended inside an AI-generated answer. The two overlap in some inputs but differ in goal and measurement — in Ahrefs' July 2025 test, only about 8% of ChatGPT's in-text citations ranked in Google's top 10 for the same query, so strong Google rankings do not indicate AI recommends a practice.
How much should a real AEO engagement cost?
AEO audits typically run $1,500-$12,000 depending on scope; ongoing retainers range from $1,500-$15,000+/month. A fixed-price audit around $5,000 covering platform coverage, doctor/service/location visibility mapping, directory consistency, and a competitor comparison is a reasonable benchmark. Pricing of a few hundred dollars a month for "AEO" is a strong signal the work is rebranded SEO.
What should I ask my agency before renewing a contract?
Ask directly: "Which of my doctors does ChatGPT recommend for [service] in [city], and which of my locations show up in AI answers?" An agency that cannot answer this by doctor, service, and location is not delivering AEO — regardless of what the invoice calls it. See our full questions-to-ask checklist before the renewal conversation.
