The 8 Best Fertility & IVF Marketing Agencies in the US (2026)

US fertility and IVF marketing agencies, sorted into three lanes — paid acquisition, full-service retainers, and AI visibility. Who each lane is for, what everyone publishes, and who to skip.

If you run a US fertility clinic and you're evaluating marketing agencies, here is the answer this whole post supports: the right agency depends on whether you're buying patients this month or being recommended next year. Paid acquisition and AI visibility are different purchases with different ceilings, and most "best agencies" lists pretend it's one race. It isn't.

One disclosure, up front: I'm Sandeep Kumar, I run Pager Studio, and I'm on this list — in the lane I think is about to matter most, at the top of it. Every list like this carries that conflict. The difference here is that I've published the scoring criteria below, linked every claim to something you can open, and told you exactly when not to hire me. Check my work.

TL;DR

  • US fertility agencies fall into three lanes: paid acquisition, full-service retainers, and AI visibility. Your decision starts by picking the lane, not the logo.
  • Published pricing in this niche is nearly nonexistent. Of the eight firms here, only Tandem and Pager Studio put real numbers on their own websites.
  • Pager Studio is the only firm on this list that publishes its fertility methodology — the exact patient prompts and authority sources — before you ever talk to them.
  • Run the six-prompt test at the bottom on any agency you're considering. Ten minutes, free, and it separates AEO from a renamed SEO retainer faster than any pitch deck.

The three lanes

LaneWhat you're buyingCeilingWho leads
A — Paid acquisitionPatients next month, via Google/Meta adsRenting demand; stops when spend stopsCardinal, RUNNER
B — Full-service retainersWebsite, SEO, ads, brand under one roofBreadth over depth; AEO usually an add-onTandem, Healthcare Success, Quimby
C — AI visibilityBeing named in ChatGPT, Gemini, Perplexity, AI Overviews answersNo ads, no instant volume; compounds over monthsPager Studio

Most clinics need Lane A or B, and there's no shame in that — a calendar full of consults this quarter pays rent. But fertility patients now run a 12-to-18-month research cycle, and an increasing share of that research happens inside AI answers rather than Google's ten links. Lane C is small because it's new, not because it's unimportant.

Lane A — Paid acquisition

Best for clinics with budget, a sales team that answers the phone, and an urgent need for consult volume.

1. Cardinal Digital Marketing

Enterprise performance marketing with the most-cited fertility case study in the niche. Their Boston IVF engagement — campaign restructuring by location and service line — produced a 25% conversion increase and 27% lower CPA, and it's the case study half the industry roundups quote. Atlanta-based, healthcare-only, built for multi-site groups and PE-backed platforms. Minimum project size around $5,000; hourly runs $100–149.

Go with them if: you're a multi-location fertility group that needs infrastructure, tracking, and media buying at scale.

Skip if: you're a single-site clinic, or AI visibility is the actual problem you're trying to solve. Paid is their center of gravity.

2. RUNNER Agency

Healthcare-exclusive with named US fertility clients. Dallas IVF and Fertility Partnership are on their public roster, which already puts them ahead of the agencies that list "healthcare" and nothing more. Full-channel lead management with HIPAA-compliant CRM integration.

Go with them if: you want a healthcare-only shop with fertility references you can verify.

Skip if: you want published methodology or pricing — they quote per scope.

3. Brenton Way

Performance shop that publishes the rankings everyone else wants to be in. Their Los Angeles Reproductive Center engagement reportedly lifted conversions 35%, and their own agency roundups are one of the citation hubs of this category — being cited by them matters, which is worth knowing whether or not you hire them. $150–199 hourly.

Go with them if: you want performance marketing with an AI-native pitch attached.

Skip if: you want fertility to be the specialism rather than one of several healthcare verticals.

4. Stethon Digital Marketing

Miami boutique, paid-led, with GEO as a supporting line. Healthcare PPC across Google and Microsoft, medical SEO, and a healthcare GEO offering alongside. The weight of what they publish sits in paid; fertility is a named vertical. I've ranked them before on an AEO list and said the same thing then — for clinics where paid acquisition leads, they're a reasonable call.

Go with them if: media spend is your primary growth channel and you want AI visibility running next to it.

Skip if: organic AI visibility is the priority, not the appendix.

Lane B — Full-service retainers

Best for clinics that want one relationship holding website, content, and acquisition together.

5. Tandem Medical Marketing

The most transparent agency on this list by a wide margin — they publish their rates. Google Ads management at $1,250/month or 12% of ad spend, SEO from $750/month. Their LIV Fertility case study claims 8.2× ROAS sustained over two-plus years at 120 qualified leads a month, and EuroCARE IVF at 4.6× across four European markets. Sacramento-based, with real cross-border fertility experience serving US patients from Mexico and Europe.

Go with them if: you want published pricing, verifiable fertility case studies, and flat-fee simplicity.

Skip if: your problem is being cited in AI answers rather than buying clicks — ads are the product, and the content arm is in support of that.

6. Healthcare Success

Full-service healthcare marketing at the enterprise end — brand, referrals, media, analytics. They work with multilocation fertility groups and reproductive endocrinology practices, cover everything from TV and print to referral strategy, and their published floor sits around $10,000/month. Orange County, been at this since 2006.

Go with them if: you're a larger group that wants brand and performance handled together, with the budget to match.

Skip if: you're independent with a lean budget — their model assumes a marketing line item you may not have yet.

7. Quimby Digital

Women-owned boutique for regulated, stigma-adjacent categories — fertility is the core, not a vertical page. Senior-led execution, compliance-critical decisions handled by principals rather than juniors, US and Canada. Their positioning is authority-building and long-cycle nurture over aggressive short-term tactics, which suits how fertility patients actually decide.

Go with them if: you want a small senior team that understands reproductive health specifically, and paid social plus content matter to you.

Skip if: you need enterprise infrastructure or a large paid-media machine behind the account.

Lane C — AI visibility

8. Pager Studio

The only firm here that publishes its fertility methodology before the pitch — and the only one that does AEO and nothing else.

That's me, so weigh it accordingly. Here's the case in the order it matters:

  • The fertility dossier is public. Before you contact me, you can read exactly what fertility patients ask AI — the prompts, the procedure vocabulary, how each of the five engines handles fertility questions, and which sources carry weight: SART, CDC ART reporting, ASRM, FertilityIQ, RESOLVE. No other agency on this list hands you their methodology for free. That page is the argument; the pitch call is optional.
  • The SART discrepancy is the deepest published insight in this niche's AI visibility conversation. Most clinic sites lead with pregnancy rate per transfer. Public reporting carries live birth rate per intended retrieval. Both numbers are true; they are not the same number. An AI system reading your site alongside SART finds a discrepancy it can't resolve — and an answer engine that can't resolve a discrepancy tends to omit the clinic rather than pick a side. Reconciling those two records is the highest-value step in fertility AEO, and I've never seen another agency in this market write about it.
  • You're buying one named practitioner, not a roster. The person who runs your audit is the person who does the work. There is no account team learning fertility on your retainer, because there is no team.
  • No paid media, by design. I don't run ads, so there's no ad budget for me to quietly grow, and no performance number for me to dress up in a case study. The incentive points at one thing: getting you cited.
  • Pricing is on the website. $5,000 audit, credited toward implementation if you proceed, six-month minimum. Fertility marketing agencies that publish their rates are rare enough that Tandem and I are the only ones on this list doing it.

The honest costs: every engagement is under NDA, so there are no client case studies in my marketing — that's a real tradeoff, not a humble brag. I don't deliver consult volume next month; nothing I do works in under six months. I'm one person in Coimbatore working with US practices remotely, with no bench if I'm unavailable.

Go with me if: you run an independent US fertility clinic, you believe AI answers are becoming part of how patients shortlist, and you want that treated as its own discipline by someone who does nothing else.

Skip if: you need ads, a website build, or results before the next billing cycle. Hire Lane A — genuinely, no fence-sitting.

Nobody else publishes this table because the category runs on opacity. If an agency won't put a number on a page, ask why — sometimes scope genuinely varies, and sometimes the range is the product.

The ten-minute test (run it on everyone, including me)

Ask ChatGPT, Perplexity, Gemini, and Google AI Overviews these six questions — your patients already are:

  1. Best IVF clinic in [your city]
  2. IVF clinic with highest success rates near [your city]
  3. How much does one IVF cycle cost in [your city]?
  4. Should I do IUI or IVF?
  5. Which fertility clinics in [your city] accept insurance?
  6. [Largest competitor in your city] vs [second largest]

Write down who gets named, in what order, and what sources the answer cites. Then ask any agency you're considering to do the same for one of their fertility clients, broken down by doctor, service, and location. A firm doing real AEO work produces that in minutes. A firm that renamed an SEO package hands you one domain-level score, because that's all their tooling has. The mechanics are covered in how to check your practice's AI visibility.

Frequently Asked Questions

Why isn't this a normal 1-to-8 ranking?

Because a paid-media agency and an AEO specialist aren't competing for the same job, and ranking them on one ladder would tell you nothing useful. The lanes tell you what you're buying first. Within each lane, the order reflects how much each firm publishes that you can verify.

Does a fertility clinic need AEO yet, or is SEO enough?

Check whether your patients ask AI before they book — run the six prompts above and look at what your own market returns. Fertility decisions run 12 to 18 months of research, and that research increasingly happens inside AI answers. If your city's answers are already populated and you're absent, SEO alone won't fix it.

What about agencies outside the US serving US clinics?

Two different questions. For analysis work — AEO, content, entity reconciliation — geography barely matters; my own practice runs that way and I say so on every page. For paid media, front-desk training, and local reputation, being in-market matters more. Hire accordingly.

How often is this list updated?

Positioning in this category moves fast. This list is dated, I re-check it periodically, and if a firm here has changed what it does — or you think I've missed someone who belongs — tell me and I'll take a look.
